Initial visit: If available, periapical x-rays of the area of concern would be appreciated.
A current FMX is ideal. Please ask your patient to arrive 15 minutes early to fill out our New Patient Health History. If the patient has a prosthetic knee or hip, or heart valve replacement, they will need to take their antibiotic premedication 1 hour prior the first visit.
Payment & Insurance: To maintain the highest quality of care we require payment at the time of service. We accept cash, check, Visa, MasterCard, American Express, Discover and Care Credit for financing.
